A Telecommunications Project must be initiated when working with Facilities Management to complete construction and/or a remodeling project that requires network & telecommunications installations or changes.
A Telecommunications Project is not required for minor changes within your department, such as activating a single jack or modifying your phone service. To request these types of changes, please complete this form.

Step 1: Complete the Project Authorization form
Please when completing this form to include the Facilities project number, your departmental fund and cost center, drawings (if available), and primary contact.
Attach the completed form to your Service Hub work order (Step 2)
Step 2: Submit a Service Hub work order
Once the work order (with Project Authorization Form attached) is received by the Telecommunications team, we will schedule a walk-through if necessary and provide the final quote.
